This is results of care at St. Peters Hospital and she was in the most severe pain imaginable because her hip was dislocated from this hospital which they failed to inform me till the last day of discharge. An Ortho. Dr. said she had this since August 2014 while under their care (see X-ray)

My mother came to this Rehab Center in January 2016. At the time, she was dealing with one stage 4 Bed Sore. In the pictures to the left, it shows she got 5 Bed Sores now to deal for incompitent staff to properly rotate her every 2 Hours as needed to prevent this.

Other Issues of gross negligence they did to her as listed below:

2-23-2016 Madison Center sends Patricia Tanasy to the ER and addmitted for Hemagloben off.

3-31-2016 Madison Center sends Patricia Tanasy to the Saint Peters ER and had to have Blood Transfusion and addmitted to ICU.

5-15-2016 Madison Center sends Patricia Tanasy to Saint Peters ER for failing to regulate her sugar levals which went over 500 and a Fever.

My mother Patricia Tanasy was sent to Raritan Bay Medical in Old Bridge, NJ from Golden Living about on July 17, 2015 for Urinary Track infection from Golden Living and while she was there, A Orthopedic Surgeon calls to tell me that my mother has left knee dislocated and states they are unable to fix knee and sends her back to Golden Living Long term nursing home in Old Bridge.

On Friday August 14, 2015 - took her to see her Orthopedic Surgeon at Mercer Bucks Orthopedics for Doctor Ast to see about her dislocated Left Knee replacement joint. He said the condition was one of the worst he has ever seen and if my mother does not go to Hospital immediately, she could lose her leg from infection from dislocation. She went to Capital Health Hospital in southern NJ and they wanted to amputate her left leg because of infection from complications from dislocation of replacement knee which was also loose from abuse to my mother when it was dislocated. They sent her back to Golden Living and her Doctors gave Anti-Biotics. My mother is sent to Saint Peters ER from Golden Living because sugar over 535. She is hospitalized and her doctor was Dr. Tadros - Female - General THEY RELEASE HER BACK TO GOLDEN LIVING ON THURSDAY 10-01-2015

10-09-2015 Late Friday night She is Re-admitted to Emergency Room from Golden Living again.

November 23, 2015 - Golden Living sends Patricia Tanasy back to St. PETERS EMERGENCY ROOM. They admitted her. Dr. James Monica Orthopedic Surgeon operated on her left knee on 11-24-2015 to clean out infection and had to remove implant that was severely damaged and loose causing complications with infection from dislocation and put spacer in and sent her back to Golden Living again. Dr. Maganti Internal medicine for my mother. They Gave blood transfusion on 11-27-2015 because Red blood cell count was off.

Dec. 28, 2015 Golden Living sends my mother back to Saint Peters Emergency Room and they had to operate again because it was damaged again and put spacer in leg and has Mrsa. Dec. 30, 2015 - She has surgery again on left knee. They also told me she was malnourished. Jan. 04, 2016 She has surgery to clean leg again Jan. 09, 2016 – She is still in Hospital for MRSA infection to Left Knee.

	(Victoria Health) After being transferred from St. Peters Hospital to Victoria Health in Matawan NJ in 9-2014. (See Receipt) These Eye Glasses were stolen from Patricia Tanasy and they preferred to have her suffer without them for a few months and they still failed to replace them for a disabled senior citizen which is extremely sadistic. (Opinion: Avoid like the plague!)

	(Victoria Health) After being at Victoria Health in Matawan NJ in September 2014 to December 2014. This is the condition of Patricia Tanasy after she was taken to Raritan Bay Medical Center from Victoria Health Matawan. She was abused by lack of care causing her Diabetes to go over 400 which caused a blood vessel in her left eye to burst & a stage 4 Bed Sore with large-scale loss of tissue. She got so weak that she now has to eat pureed food and be fed. She can not even swallow her pills because she got so weak. She also got E-coli & ESBL infection from Victoria Health. If you care for your love one, never bring them to this horrible place. (Opinion: Avoid like the plague!)

This Bedsore is actually worse then before because Victoria Health let her just stay in bed without taking care of her causing to get so weak that she can only eat pureed food and be spoon fed.
